Hospital Discharge Checklist
Leaving the hospital is a major transition. A little preparation can help the first days at home feel more manageable.
Understand discharge instructions. Ask nurses or doctors to explain medications, activity limits, follow-up appointments, and warning signs to watch for.
Confirm follow-up care. Schedule primary care or specialist visits before you leave if possible.
Arrange transportation home. Plan who will drive and whether equipment or assistance is needed entering the home.
Prepare the home. Stock groceries, clear pathways, set up a comfortable resting area, and place frequently used items within easy reach.
Clarify medications with medical providers. Use the hospital's list and confirm timing and purpose with a pharmacist or physician—not home-care staff.
Line up additional support. Meal preparation, personal care, and companionship at home can help while strength and routines return.
Know who to call with questions. Keep phone numbers for physicians, pharmacies, and home-care contacts accessible.
